Step1.
effective dose=Cr x 20算出初始劑量

IV onset 10-20 mins
Duration 4-6 hr
4-6 hr後觀察UO，比較intake和output是否相稱
if not而且差很多，將劑量double(max 200 mg, speed<4mg/min )
due to S/E: nephrotoxicity,
ototixicity
Step2.
Frequency 120 mg Q6h-Q1.2h F/U K+ 避免太高
Exercise:
DM Nephropathy Cr=5 with dyspnea, pulmonary(around 2-3L) edema 7pm
作答
Lasix 100 mg, urine=300 at 11pm(不適合，應直接開double 提高劑量為200 mg)
